The primary objective of the County’s Whole Person Care (WPC) Program is to effectively coordinate the physical health, behavioral health, and social service needs of the most vulnerable, high-risk, resistant individuals experiencing homelessness in San Joaquin County.

Please note that before referring a client to WPC, make sure that the client meets at least one of the following WPC Populations of Focus.
- Individuals who are experiencing homelessness AND have at least one complex physical, behavioral, or developmental health need with inability to successfully self-manage.
- Individuals who are experiencing homelessness, are transitioning from incarceration, or transitioned from incarceration within the last 12 months AND have at least one additional complex need and
- Individuals experiencing homelessness who meet eligibility criteria for SMI & SUD.
